JOB DUTIES

• Treat home health pediatric patient for rehabilitative and functional status.

• Provide feedback about progress on the patient’s/client’s status to assist the physical therapist to evaluate the outcomes of care on a regular basis.

• Provides interventions for deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.

• Uses infection control measures that protect both the staff and the patient (OSHA).

• Coordinates services. Informs the physician and other personnel of changes in the patient's/client’s condition and needs.

• Demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.

• Supervises, teaches, and provides clinical direction to other physical therapy assistant personnel as required or requested.

• Promote agency philosophy and administrative policies to ensure quality of care.

• Provides effective communication to patients/clients, their family members, team members, and other health care professionals. QUALIFICATIONS

• Graduate of an accredited Diploma, of Associates and / or Bachelor’s degree

• One year of experience in a pediatric clinical care setting preferred

• Pediatric Home health experience preferred

• Reliable transportation
