Location: Mississippi (Must reside in territory)

Travel: >50% | Mobile Role

Salary Range: $55,350 - $88,560

Compensation Type: Salary + Bonus Potential

Vaccination Requirement: Annual influenza vaccination required

We’re currently seeking a Community-Based Care Manager to join our growing team of mission-driven professionals who believe everyone deserves equitable, high-quality care. If you're a licensed nurse or behavioral health professional passionate about whole-person care, we want to hear from you.

What You’ll Do:

You’ll be the trusted connector between our members, their providers, community supports, and our care teams—ensuring that each member receives culturally competent, person-centered care tailored to what matters most in their lives.

Core Responsibilities:

  • Build strong, professional relationships with members in their homes, community agencies, hospitals, or via phone/video.
  • Complete trauma-informed, strength-based assessments with a focus on social determinants of health and health equity.
  • Develop and maintain individualized care plans with an interdisciplinary care team (ICT).
  • Identify and resolve barriers to care while connecting members with community, behavioral, and physical health services.
  • Provide health education and coaching that empowers members to manage their own care.
  • Support members through care transitions and ensure timely coordination between health systems and community resources.
  • Serve as a rotating on-call clinician for the Behavioral Health/SUD Crisis Line (if licensed in behavioral health).
  • Document care plans, services, and progress using internal systems in accordance with clinical and ethical standards.

What You’ll Need:

Education & Experience:

  • Required:
  • Nursing degree or a Bachelor’s degree in a health-related field
  • Active, unrestricted RN, SW, or LPC license in Mississippi
  • 3+ years in nursing, counseling, social work, care coordination, or community health
  • Preferred:
  • Master’s degree in a health field
  • Medicaid/Medicare managed care experience

Licensure:

  • Current unrestricted clinical license (RN, Social Worker, or Clinical Counselor)
  • Valid driver’s license, personal vehicle, and insurance (required for mobile visits)

Skills & Knowledge:

  • Deep understanding of health disparities, HEDIS measures, and care management standards (CMSA/NCQA)
  • Strong interpersonal and communication skills, especially with diverse populations
  • Proficient in Microsoft Office (Outlook, Word, Excel)
  • Knowledge of public health systems, community resources, and Medicaid/Medicare benefits
  • Committed to ethical practice and trauma-informed, culturally responsive care
