Responsibilities

  • You’ll also have the structure and support you need to focus on what matters most: your patients
  • As a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care
  • You’ll work independently in the field - but never alone
  • Deliver skilled nursing care in accordance with the established plan of care
  • Monitor and assess patient conditions during home visits or shifts
  • Communicate changes in patient status to the Clinical Supervisor
  • Provide patient and caregiver education regarding medications, treatments, and disease management
  • Accurately document patient care and clinical observations
  • Coordinate care with other healthcare team members as appropriate
  • Maintain compliance with agency and state guidelines
